Candice has a real mean streak, and she's not above kicking a man when he's down. But she would have to have found out Nathan's role in the whole thing. And to make him see her illusion, either she's following him around, or she learned how to do it at a distance. Also, if someone is making Nathan see things, that someone does not have to be WTTB.
My initial thought is that Nathan's reflection shows his guilty conscience. But, since I spent episodes 11 - 19 of season 1 thinking Peter's dream was metaphorical, I'll go with someone like Candice casting an illusion on Nathan.
How about this---Candice was helping Linderman, because she wanted to punish the world for its attitude toward beauty/appearances. She is angry that the plan was foiled by Nathan and begins to exact revenge by making him think that he is disfigured. To Nathan, the image we saw in the mirror is his reality. Candice could also be WTTB. That would explain Molly's dreams (also a perception).

To go along with that, maybe after
Sylar kills Candice next week (if that does come to pass), the illusion will go away, which is why it was revealed in a recent spoiler that Nathan will be shaving.
However, it also said that he asks Simon & Monty what they think about his beard, which implies that the beard isn't an illusion. Only a week until we find out! But I'm happy enough waiting 1 more hour for episode 2. Lol.
Well, even if his kids asked him, it could still be an illusion. I mean, Sylar was fooling the entire world for a good while. Remember our heated debate about how the cameras wouldn't pick up the bending of light and blah blah....? lol that was hilarious. but seriously, if it IS an illusion, then nathan's kids would probably be fooled too IMO.
I'm going with illusion. Unless the writers can think of a clever way to show us that Nate's face didn't get burned, which I'm sure they could.
Either that, or he scalped Emo Peter before he blew up and glued it to his face, which is why Peter has no hair now:

It could have been anyone. I'm sure there is more than one healer, and definitely another empath who might have the power. You may not want to admit it, but duplicate powers are happening, and we know empathy is one of them.
